live active Referral Scheme

Many people believe that only intense physical activity will benefit their health. This is wrong. The greatest health gains come when inactive people start to do moderate activity that adds up to 30 minutes a day. If you can do more that's great, but it is that 30 minutes that will make the real difference.
Being active is something you should enjoy and be able to fit in to your daily routine - on your own or with others. Moderate activity should make your heart beat faster, breathe more quickly and feel warmer. You should still be able to hold a conversation.
PKL have a wide range of options available to you ranging from local health walks to supervised gym or activity classes. We also accept that some people need more help to get started and the live active Activity Referral Programme can provide that assistance.


SUPPORTED REFERRAL
This programme provides support for individuals meeting the medical criteria of the programme. Your GP, Health Visitor or other Health Professional can offer you this programme. It involves:

  1. Referral to the Activity Referral Programme by your Health Professional based on your needs.
  2. A one to one consultation with a qualified exercise consultant.
  3. An appropriate health and fitness check.
  4. An activity programme designed for you.
  5. 24 visits over 12 weeks to supported / supervised activity. (Gym, Pool or Circuit Class)
  6. On site support and advice
  7. Signposts to more advice and support.

Sessions cost: £2.90 live active / £2.40 Concession & Senior / Free Spectrum & LA Access

What to do now?
If you have not already done so, contact your Doctor, Public Health Nurse / Health Visitor, Practice Nurse or District Nurse, to discuss the best way forward. Your surgery will advise you who to make an appointment with.

If Supported Referral is appropriate, you will be given an 'Activity Referral Form' and relevant information. Your Health Care Practioner will send your referral to PKL and we will contact you to discuss your needs and agree which facility you will use.
Centre staff will book your initial consultation appointment and you will be given all the necessary information to prepare you for your visit.
Consultation: the exercise professional will discuss your goals, carry out some basic appropriate health checks and agree with you a physical activity programme, tailored to your specific needs.
Activity Programme: You will be supported for 12 weeks to achieve your goals. Ideally you should attend twice a week for most benefit. After a final consultation you will be encouraged to progress on to other activity opportunities either independently to through our continuation pathway.

The Supported Referral Programme is aimed at those with low / medium risk medical conditions. If at your consultation we identify that you may be more suited and able to exercise indendently you could benefit from reduced cost access to our programmes for 3 months. Your consultant will discuss your needs and advise you accordingly.

The live active Referral Programme offers a wide variety of activities to suit your needs. These opportunities take place across Perth & Kinross and cater for all abilities. Choices include Social Aerobics, Aquafit, Badminton and Yoga. As well as programme of led health walks as part of the Stride for Life programme. The aim of the programme is to provide affordable and accessible opportunities for those who will benefit the most from adopting a more active lifestyle.

To take part in the scheme you must get a live active card holder. Everyone can get a live active Card which provides discounted rates for all PKL activities. With an annual cost of only £5 for adults (free for all other categories, including concession adults) the card represents great value.
