Membership / Live Active Card Application Form

If you wish to apply for a Live Active Card or a Membership, you can download a copy of the application form by clicking the link on the right.  Please note that you are unable to complete the form online.  The form must be printed off and returned to a Live Active Leisure venue.

Please note, if applying for a Joint or Family Spectrum Membership, only 1 application form needs to be completed.

Changes to your Membership

The following forms are available for download, alternatively forms can be obtained from your local venue, by email or by telephoning 01738 492440. Please note, we require 14 working days notice to process all changes to your membership.

Membership Holiday Form
Do you exercise seasonally?  Are you a student going back to Uni?  Or have you recently suffered an injury?  The flexibility of our Membership allows members to suspend their payments from 1 month to 6 months. Suspended memberships can then choose when to restart their membership.  Memberships will automatically restart after 6 months.

Change to Subscription and Bank Details Form
Looking to upgrade or downgrade your Membership or have you changed your bank account?  Complete this form and let us know what you want to change too.  For example Single to Joint or Membership to Membership+ Power Plates.

Membership Cancellation Form
This form must be completed in order to cancel your Membership.  14 working days notice is required for all cancellations.
